Pick up my pet(s) from boarding Make decisions for my pet(s) on my behalf in an emergency

This contact must be willing and able, in case of an emergency, to pick up or release ownership of one or more of your pets.

Emergencies include:

• Your dog is over 40lbs and has bitten me and/or shows dangerous aggression, will not allow me to handle/feed/walk for a time period of more than 24 hours. If your emergency contact is unwilling or unable to collect said pet, an additional fee of $20 per day will be due, payable immediately. (Video evidence would be offered)

  
  

• Your pet shows symptoms of a contagious disease (will go to Vet for exam/confirmation), if your contact refuses to pick up the pet, it will stay at the vet at your expense and you will be asked to call the vet to arrange treatment. If you cannot be reached, your back up will be notified.

• You are unable to pick up your pets or extend service more than 24 hours past your reservation I have not been contacted by you and you don’t show up for pick up. I cannot reach you by phone. I will contact your back up to see what might be keeping you and what will become of the pets.

• Medical emergency concerning your pet

To bring and not to bring:

• Food, portioned or with measuring container, PLEASE bring some treats, chews….(Comfortable beds, blankets, Nylabones and toys, food bowls are provided.)

• Medication, if any, in prescription container

• Dogs will not be given rawhide or tennis balls while here

Note that ANY items you leave here can potentially get damaged or lost. This includes leashes, beds, bowls, toys….

Emergency Medical

This is the amount you accept to repay, that I will use from my own funds for emergency treatment until you can be reached. You agree to repay this amount before picking up your pet. The maximum amount is $500 for dogs and cats. With your submission you agree to authorize Katja Killingsworth to seek veterinary aid and treatment as needed and as detailed in policy (see policies page and e-mail response received)

Please note, if the pet is suffering and neither I nor Veterinarian staff can reach you or your emergency contact within a reasonable amount of time, your pet may be euthanized, if it is the veterinarian’s recommendation. EVERY effort will be made to reach you before this would happen. Though every effort is made to keep everyone safe, dogs that play together could get injured.

Please Upload:

• Vaccination Records, basic vaccines (Distemper parvo for dogs) and Rabies Certificate (NO SHOTS DUE REMINDERS)

• If there are health issues the pet is not currently on medication for, please upload the latest Vet Visit paperwork
